Common Questions

What is a Geriatric Care Manager?

A geriatric care manager is a professional who specializes in assisting older people and their families in meeting their long-term care arrangements.

Geriatric Care Managers develop an individualized care plan, providing for immediate support and long term needs. Innovative Healthcare Consultants will arrange for services, screen and assist in hiring non-medical home care, financial and legal

geriatric care managementservices, home maintenance, meals, transportation, pet care and other services. Care managers may accompany clients to medical appointments, will coordinate services, monitor health and other care providers, periodically reassess the client and are available for telephone assurance. All this is in an effort to maximize independence and autonomy and make it possible for adults to stay at the least restrictive level of care, often in their own homes.

Why would I use a Geriatric Care Manager?

You have serious concerns about a frail or disabled adult and need information and assistance to ensure safety, independence and quality of life. You have no family living nearby. Geriatric Care Managers are here to help. Not as a replacement for family, but as an extra member on whom you can rely to advocate for you - a professional who knows how to determine needs and link you to appropriate services to make for peace of mind.

How does the process work?

An assessment is typically necessary for care managers to understand the environment that the older adult is living in, their level of functioning and their current support network. The assessment involves interviewing the older adult and family members, gathering relevant medical records, developing an individualized action plan and discussing the options with the older adult family members and caregivers.

When is Geriatric Care Management needed?

Geriatric Care Management is needed when an adult is faced with decreased capacity to function due to complications of aging, illness, disability, altered cognitive status and/or a loss of social support systems.
Geriatric Care Managers provide assistance when time and stress in providing care for a family member may conflict with the caregiver's work, family or other obligations. Care management can be an essential link in the chain to assist long distance caregivers.

Short-term/long-term support for clients and caregivers may be needed to assist in decision-making and problem-solving life issues such as: changes in physical and mental status, chronic illness, grief and loss, retirement, widowhood, relocation, family conflict, care for a family member.

How does one pay for Geriatric Care Management Services?

Most Geriatric Care Managers provide private care management on a fee for service basis but you will want to ask ahead of time about billing fees. There is usually an hourly rate for care management services. Fees are stated in written form and approved by the person accepting responsibility for payment prior to initiation of services.

What are the qualifications of care managers?

Innovative Healthcare's care managers are registered nurses and certified care managers licensed through their professional associations. At Innovative Healthcare they have a bachelor's degree or equivalent training in gerontology, social work, nursing or counseling. They are also members of the National Association of Professional Geriatric Case Managers (NAPGCM).

Are private care management services covered by Medicare, Medicaid or private insurance?
No. Currently, care management fees are typically paid by the older adult, their estate, family members or other caregivers. Some long-term-care insurance policies cover care management services.

How can professional Geriatric Care Managers save you money?

Geriatric Care Managers can help you save money in several ways. First, they can help to plan effectively for the future and assist in avoiding a crisis. Second, care managers can often arrange for services to be delivered in an older adult's home rather than requiring a costly move to a retirement or nursing facility. Third, because care managers are aware of both the needs of older adults and the available resources, they can be efficient in matching service needs with appropriate agencies.

San Diego Home Healthcare

Home healthcare covers a wide range of services delivered in the home of a client who is elderly, recovering from a medical condition, disabled, chronically ill or simply in need of assistance with the activities of daily living. Our services include skilled aides and companion services, nursing, specialized care for individuals with conditions such as Alzheimer’s disease or diabetes and relief/respite programs for caregivers. We provide our services to individuals on a long or short-term basis.

Private duty home care refers to those services that are not covered under Medicare Home Health, but that are essential to individuals with functional limitations. This is especially important for chronic care needs that are not covered under Medicare. Private duty care is often covered by long term care insurance policies.

Our staff of caregivers and companions provides the highest quality of services. Potential employees go through a rigorous screening process with background and DMV checks and live scanning. We also require proof of automobile insurance, up-to-date TB tests and we check each applicant’s personal and professional references.

Placing security and safety as a priority, our caregivers are bonded; they are also covered by worker’s compensation and liability insurances. Caregivers are oriented and monitored by an RN case manager who provides and implements the plan of care. Our selection process assures our clients peace of mind and the security of knowing they have a professional team working for them.

Our household services are designed to address convenience and safety issues in a person's home. This provides consistency in care for older people who do not tolerate change well. Services include:

Home safety planning and modification, including bathroom safety equipment and emergency response systems

• Meal preparation and diet management
• Light housekeeping
• Scheduling home maintenance tasks
• Errands
• Accompaniment to appointments, shopping, places of worship, restaurants, movies and more
• Identifying technology and systems to assist people with the safest management of their individual disease conditions

We pay particular attention to detail in every interaction with our clients. Caregiver coordinator or case manager personally introduces and orients every caregiver to each client to ensure that the caregiver understands the needs and preferences of the client.

Emergency care for unplanned situations

When appropriate, a companion or home health aide is selected to provide direct care services for the individual. Companions and home health aides are available on an hourly or live-in schedule. Our staff is available on a full-time or as-needed basis 24 hours a day, 7 days a week. Our in-home caregivers are trained and supervised by our nurse care managers. There are a number of tasks and activities our care giving staff can provide, including, but not limited to, the following:

• Assistance with daily activities such as grooming and dressing, cooking and eating, bathing, toileting and moving
• Help with activities such as shopping, driving, housework, laundry, using the phone, driving to doctors' visits and taking medications
• Socialization and companionship
• Facilitating special interests such as the arts, languages, and accompanying to social and sporting activities
• Assistance while traveling

Chronic health issues create a very stressful situation for both the care recipient and the family. Our specially trained Care Managers interact with the care recipient, family, friends, and other members of the community to help mediate conflict, keep family members informed and involved, and encourage social stimulation for the older person. Areas of focus include:

• Education to help everyone understand what is happening plus how to best meet the needs of all family members
• Help with problem solving around options for care
• Discussion support to insure that families are making the best choices they can for their individual circumstances
